In order to validate your certification as a Lumen Care Laboratory Vibratory Reinformation therapist, you must provide a complete treatment follow-up sheet (right to the end of the box) for 2 to 3 clients.

The plan for the follow-up sheets for each person should be broken down as follows:

1. A summary of the medical history
2. The result of the sensitivity test
3. A note on the organisation of reagents and treatment planning, which has been drawn up
   
4. End-of-process validation (rates recorded - options taken for full validation if not effective)
5. Customer comments (for each treatment) :
   - Feelings during treatment
   - Benefits gained / benefits expected
   
6. Practitioner's comments
   - The proposed follow-up
   - Brief summary of the treatment as a whole
